- Has 20 or more deficiencies within the last 12 months confirmed by the Port State Control Reports

- Document claiming applicant’s competency for ship registration
- Copy of ship’s sale and purchase agreement or subsequent bill of sale
- Copies of all valid certificates issued by Recognized Classification Society or the letter of approval from Recognized classification society
- Copy of certificate of registry issued by the previous flag state
- Affidavit from the owner confirming that the vessel will be deleted from the previous flag for permanent registration under Georgian flag
- Copy of certificate of company or certificate of incorporation of a company if the shipowner is a juridical person; in case of individual owner – copy of ID
- Statement concerning satisfaction of hypotheque from the creditor
- Payment for service fees and other related charges
- Completed application form for permanent registration
- Completed application form for ship station license
- Document claiming applicant’s competency (whether natural or juridical person) for ship registration
- Original of ship’s sale and purchase agreement or subsequent bill of sale
- Copies of all valid statutory certificates issued by recognized classification society
- Original deletion certificate from previous flag
- Copy of required mandatory insurance certificate
- Original ship inspection report of surveyor approved by MTA
- Two recent colored photos of the ship (front and side view – port of registry to be seen)
- Copy of continuous synopsis record
- Statement concerning satisfaction of hypotheque from the creditor (if applicable)
